What This Test Measures

A standard lipid panel includes total cholesterol, LDL, HDL, and triglycerides. TRT has variable effects on lipids: it typically lowers HDL by 5-10%, may slightly lower LDL, and often improves triglycerides through better metabolic health and body composition. The HDL reduction is generally modest and may be offset by other cardiovascular benefits of optimized testosterone.

Aromatase inhibitors can further reduce HDL, which is another reason to minimize their use.

Why This Matters for TRT

TRT mildly reduces HDL (5-10%). Monitor especially if using an aromatase inhibitor, which can further worsen the lipid profile. Target: HDL >40, LDL <100, triglycerides <150.
