What This Test Measures

Estradiol is the primary estrogen in men, produced mainly through aromatization of testosterone. The sensitive LC-MS/MS assay (not the standard immunoassay designed for female ranges) is essential for accurate male estradiol measurement. Estradiol is important for bone density, cardiovascular health, libido, and cognitive function.

Both too-high and too-low estradiol cause problems. Symptoms of high E2 include water retention, mood swings, and gynecomastia. Symptoms of low E2 include joint pain, fatigue, and low libido.

Why This Matters for TRT

Must be measured with sensitive assay (LC-MS/MS). Target 20-35 pg/mL on TRT. Manage with injection frequency first, AI only when E2 is high AND symptomatic.
